Zelle Payment Required to Complete Registration

To secure your child’s spot in the For The Students Physical Education Program, payment must be submitted via Zelle after completing this registration form.

In the memo please include: Child’s First & Last Name + Month/Session Type (Example: “Child's Full Name/December/Twice Weekly”)

Your registration will be considered pending until payment is received.

Once payment is submitted, you will receive a confirmation message within 24 hours.

Photo & Video Consent

I acknowledge that DEV Sports Corp may take photos and videos of my child during PE and sports enrichment classes. By providing consent below, I understand that:

• Photos and videos may be used on DEV Sports Corp’s website, social media, or promotional materials.

• No personal identifying information will be shared.

• No payment or compensation will be provided for the use of these images.

• I may revoke this consent at any time by contacting DEV Sports Corp; however, materials already published cannot be removed.
